The best pillows for side sleepers with shoulder pain

By KevinUpdated 30 min read✓ Research-based · sources cited

The short answer

Get a pillow with a shoulder cutout and adjustable fill. Our top pick is the Coop Original Crescent, which curves around the down shoulder. Sleep on your pain-free side and hug a second pillow to support the top arm. If you sleep with an arm under your pillow, the Eli & Elm cutout is the most-credited fix in owner comments.

The Crescent uses the same tunable fill as the Coop Original, cut into a curve so the down-side shoulder can tuck in close instead of pushing the pillow away. Sleep Foundation names it Best Value in its side-sleeper guide. The curve also helps people who roll onto their backs. The trade-offs: it's an odd shape for standard pillowcases, and it has a shorter track record than the Original.

Pros

  • Crescent curve leaves room for the shoulder, which helps side sleepers with shoulder pressure
  • Same adjustable fill system as the bestselling Original
  • Designed for side sleepers who roll onto their backs
  • 5-year warranty

Cons

  • Unusual shape may not fit standard pillowcases (Coop sells a matching case)
  • Fill tuning takes trial and error
  • Possible initial foam odor (reported for the Original fill)
  • 100-night trial covers orders from Coop, not Amazon

Eli & Elm is built only for side sleepers. A curved cutout on one edge makes room for the down-side shoulder, and springy shredded latex keeps the height from collapsing overnight. Trials give latex only modest support for neck pain, so the cutout and adjustable height are the main reasons to buy it. Sleep Foundation names it in three separate guides. Owners who sleep with an arm under the pillow like it most. Common complaints are ear pressure, a break-in period and its weight (about 6.4 lb).

Pros

  • U-shaped cutout lets the shoulder tuck in, keeping the neck aligned
  • Latex fill is responsive and sleeps cooler than solid memory foam
  • Loft adjusts by removing latex noodles
  • Repeated Sleep Foundation picks for side sleepers and neck pain (2026)
  • Washable cotton-blend cover

Cons

  • Heavy, at 6.4 lb (Queen) per Amazon
  • Some buyers report odor, initial firmness and durability concerns
  • Short 45-night trial: Eli & Elm wants two weeks of use first and deducts return shipping
  • Not suited to people with latex allergies

Layla mixes shredded memory foam with kapok, a silky plant fiber, so the pillow is lighter and squishier than a typical shredded-foam pillow. You can mold it into the shoulder gap and remove fill to fine-tune it. Sleep Foundation names it best for pressure relief in its neck-pain guide. Owners praise its comfort and moldability, though testers say it starts out firm and some owners find it dense. Only the cover is washable; the fill is spot clean only.

Pros

  • Kapok and foam blend feels lighter and fluffier than pure shredded foam
  • Adjustable and moldable for side, back or combo sleeping
  • Several 2026 Sleep Foundation and Sleepopolis awards for side sleepers, neck and shoulder pain
  • CertiPUR-US foam
  • 5-year warranty on seams and cover fading (not flattening)

Cons

  • Only the cover is washable; the inner pillow is spot clean only
  • Temporary odor and foam particles when it first expands
  • Cooling effect is modest per Reviewed
  • 30-night trial, and Layla asks for two weeks of use first

Nest's side-sleeper version of the Easy Breather adds a crescent cutout for the shoulder and a cooling cover over adjustable foam fill with air channels. The crescent comes in one size, with no king; a separate Jr. Side Sleeper is aimed at younger sleepers and travelers. It's a strong design with a thin track record on Amazon so far, and the 30-night trial and 2-year warranty are shorter than Coop's.

Pros

  • Crescent shape built for side sleepers, with room for the shoulder
  • Cooling Icecore cover plus foam with air channels
  • Adjustable fill height
  • CertiPUR-US foam; removable, washable cover
  • Tom's Guide 2025 side-sleeper award winner

Cons

  • More expensive than comparable shredded-foam pillows
  • Short 30-night trial (returns must be unwashed, in original packaging) and a 2-year warranty that excludes loft changes
  • Tom's Guide's 2026 list ranked TEMPUR-Neck above it for side sleepers

Saatva puts a shredded natural latex core, which testers describe as Talalay, inside a soft microfiber outer chamber. You get a plush surface with latex support underneath. It comes in two lofts, 4–5 in or 6–7 in, so most frames can pick a good match, but the Amazon listing opens on Standard, so side sleepers should switch to High. It's the single most-recommended pillow across the 2026 side-sleeper guides we reviewed, including Sleep Foundation's Best Overall. The catches: it's expensive, the trial is 45 nights, and the warranty is only 1 year.

Pros

  • The most frequently awarded side-sleeper pillow across 2026 editorial guides
  • Choose Standard (4-5 in) or High (6-7 in) loft; the layers also unzip into a much lower pillow
  • Shredded latex core adds support under the plush layer and sleeps cool
  • Plush outer layer gives a down-like surface without feathers
  • Organic cotton, machine-washable cover

Cons

  • Expensive
  • Only a 45-night trial and 1-year warranty
  • Very tall in High loft, which can be too much for small-framed sleepers
  • Not suitable for latex allergies
  • Amazon listing opens on Standard Loft; most side sleepers need High Loft

Malouf's Shoulder Zoned Dough is a solid gel-foam pillow with a shoulder cutout. Its zoned columns cradle the head in the center while the firmer edges support the neck. Unlike shredded pillows, it never needs fluffing. The 6-inch loft can't be adjusted, so very broad shoulders may find it low. We found no major independent editorial test, so this is a secondary pick.

Pros

  • Shoulder cutout designed specifically for side sleepers
  • Zoned foam: soft center for the head, firmer edges for neck support
  • Gel infusion and TENCEL cover aimed at cooler sleep
  • 5-year US warranty
  • Sold and shipped by Amazon

Cons

  • No major 2025-2026 editorial review found
  • Fixed 6 in loft may be too low for very broad shoulders (one retailer review)
  • Not adjustable
  • 30-day returns apply to orders from Malouf's site; Amazon orders follow Amazon's policy

Why side sleeping hurts your shoulder

On your side, the down shoulder carries part of your body weight. If the pillow is too low, it also carries some of your head. In 20 healthy volunteers, pressure under the shoulder's bony arch (the subacromial space, where the rotator cuff runs) was significantly lower on the back than on the side or front[4]. That space also narrows when you raise your arm[5], so a down arm stretched overhead or jammed under the pillow can make things worse.

The top shoulder has the opposite problem. With nothing to rest on, the arm falls forward across your chest and drags the shoulder with it. Cleveland Clinic advises supporting that arm on a pillow so it stays straight and neutral[6].

Height matters for the muscles too. In 21 adults lying on their side, a 2-inch (5 cm) pillow raised activity in the down-side middle trapezius, across the shoulder blade, compared with a 4-inch (10 cm) pillow[7]. Find your own target in our loft guide.

Does side sleeping cause shoulder problems?

There's no good evidence that it does, but it can aggravate a shoulder that's already sore. The research is thin:

  • In a clinic series of 58 people with significant rotator cuff tears, 52 were side sleepers[8]. But there was no comparison group, and over 70% of adults in an Australian survey said they mostly sleep on their side anyway[9].
  • In 761 US workers, the positions expected to be riskiest weren't linked to more shoulder pain, perhaps because people with sore shoulders learn to avoid them[10].
  • In 83 Danish chiropractic patients with one painful shoulder, 67% of those who slept on one side lay on the painful one[2]. Which came first is unclear.

So you don't need to give up side sleeping. You need to take load off the sore shoulder and give both arms somewhere to rest.

Why shoulder pain is worse at night

Night pain is a classic shoulder symptom, and it disrupts sleep for most people who have it. OrthoInfo, the American Academy of Orthopaedic Surgeons' patient site, lists night pain as typical of rotator cuff tendinitis, rotator cuff tears and shoulder arthritis[5,11,12]. Nobody knows exactly why. A Cleveland Clinic physical therapist points to blood flow, muscle tension and a quiet body noticing pain more[13].

In 130 people with shoulder pain lasting three months or more, 81.5% had disturbed sleep, whatever the diagnosis[3]. The figure was 89% before rotator cuff repair[14] and 91% before shoulder replacement for arthritis[15]. Among 343 patients, those with frozen shoulder slept worst, though pain scores were similar across conditions[16].

The link runs both ways. Poor sleep predicts new and worsening chronic pain, and day-by-day studies find sleep predicts pain more reliably than the reverse[17]. A better night may mean an easier day.

Which side should you sleep on?

Sleep on your pain-free side or your back, and support the sore arm. A Cleveland Clinic physiatrist suggests starting on your side, but not the sore one, with a pillow between your legs[18]. Their physical therapist adds a “pillow wall” in front of you, as high as your body, for the top arm to rest on[13].

Habit is the hard part. In the Danish study, 76% of patients who shared a bed slept facing away from their partner. The authors suggest swapping sides of the bed as a simple, untested way to change which shoulder you lie on[2].

  • Both shoulders sore: lie on your back with a pillow or folded blanket under each arm so the elbows don't drop to the bed[13]. Back lying gave lower subacromial pressure than side or front lying[4]. If lying flat pulls on the shoulder, try propping yourself up on a wedge or pillows, as many people do after shoulder surgery[19,20].
  • Stomach sleeping: avoid it. It often means an arm under the pillow, which Cleveland Clinic's chiropractor calls “shoulder problem city”[6]. See our stomach sleeping guide.

Pillow features that take pressure off the shoulder

  • Shoulder cutout or crescent: lets the down shoulder slide in under the pillow edge, so the head is supported without the shoulder being jammed upward (Coop Crescent, Eli & Elm, Nest, Malouf).
  • Enough loft: a pillow that's too low shifts head weight onto the shoulder. Use the loft chart.
  • Moldable fill: shredded foam, kapok or latex can be pushed into the shoulder gap. Solid foam can't.
  • Resilient fill, not feather: in a home trial with 106 side sleepers, feather was the only pillow linked to significantly more waking shoulder-blade or arm pain than people's own pillows[21].
Feather36%
Polyester24%
Contour foam23%

Own pillow18%
Latex (solid)12%
Share of morning reports with any waking shoulder-blade or arm pain during a week on each pillow (Gordon et al. 2010, healthy adults). Only the feather result differed significantly from people's own pillows.

Treat that chart as a hint. The participants were healthy, each pillow got one week, and the latex pillow was solid, not shredded like the Eli & Elm or Saatva. The authors still recommended latex for waking shoulder-blade and arm pain[21].

Check your mattress too

A pillow can't fix a mattress that won't let your shoulder sink. Side-lying alignment changed significantly between soft and firm mattresses in a study of 25 men[22]. In an uncontrolled study of 22 people with chronic shoulder and back pain, new bedding cut reported shoulder pain by about 61% in 28 days[23]. If your bed is very firm, try a softer topper and lower your loft to match (see pillow and mattress).

How to set up the hug-pillow position

If your top shoulder hurts, hug a pillow. Cleveland Clinic advises supporting the top arm on a pillow so it rests straight and neutral, using a head pillow thick enough that the neck doesn't droop, and putting a pillow between the knees so the top leg doesn't twist your torso[6].

1 · Head pillow fills the gap2 · Hug pillow holds the top arm3 · Knee pillow levels hips
The three-pillow side-sleeping setup. Each pillow keeps a different joint from twisting.
  1. 1

    Lie on the pain-free side

Sore shoulder on top. Rest the down shoulder on the mattress, not on the pillow.

  2. 2

    Fill the head gap

Set the head pillow so your neck stays level. Push shredded fill into the space above the shoulder.

  3. 3

    Build the arm rest

Hug a pillow thick enough that the top arm stays level with your shoulder instead of sloping to the bed.

  4. 4

    Free the down arm

Slide it forward, out from under the pillow, with the elbow loosely bent, not tightly folded.

  5. 5

    Add a knee pillow

It keeps your hips level. A body pillow can do this and the hug job at once.

The elbow step matters because sleeping with your elbows bent can aggravate ulnar nerve compression[24]. For one-pillow setups, see our body pillow guide.

Arm under the pillow? Read this

Sleeping with an arm under the pillow came up in 13 of our 46 general owner comments. People usually do it because the pillow is too low. It compresses the nerves and shoulder of that arm, and it raises your head, so you need a lower pillow on top. A cutout pillow like the Eli & Elm was the most-credited fix in our data.

Why your arm goes numb at night, and when to worry

Numbness from sleeping on your arm the wrong way is common and should clear within a short time. Cleveland Clinic lists it among the simple causes of a numb arm[25]. If it keeps happening, the fingers involved point to the nerve being pressed:

  • Ring and little fingers: the ulnar nerve at the elbow (cubital tunnel). It's worse with the elbow bent or leaned on. OrthoInfo suggests wrapping a towel around the straight elbow at night[24].
  • Thumb, index, middle and ring fingers: the median nerve at the wrist (carpal tunnel). Many people sleep with their wrists bent, and a night splint keeps them straight[26].
  • Weak wrist or fingers that won't straighten: the radial nerve (“Saturday night palsy”), most often from sitting or sleeping with an arm over a chair back. It usually recovers over weeks to months[27], but get it checked.
  • Tingling through the whole arm, worse when you raise it: possibly thoracic outlet syndrome, compression of nerves or blood vessels in the lower neck and upper chest[28], or a pinched neck nerve (see our neck pain guide). Both need a clinician.

See a doctor if pins and needles don't go away[29], or if numbness keeps coming back or comes with weakness. OrthoInfo advises it for ulnar symptoms lasting more than six weeks and warns that muscle wasting from nerve compression can't be reversed[24].

Rotator cuff, bursitis, frozen shoulder and arthritis

The setup above suits most shoulder conditions, but each has its own pattern. No pillow treats any of them.

Rotator cuff tendinitis, impingement and bursitis

Inflamed rotator cuff tendons or bursa typically hurt most at night and when you reach overhead[5]. Keep the sore arm below shoulder height in bed.

Rotator cuff tears

Most come from slow wear. Risk rises after 40, and tears are more common in the dominant arm. Expect pain at rest and at night, especially lying on that shoulder, and weakness lifting or rotating the arm. Nonsurgical treatment relieves pain and improves function in about 80 to 85% of patients[11].

Frozen shoulder (adhesive capsulitis)

The joint capsule thickens and tightens. It mostly affects people aged 40 to 60, more often women and people with diabetes, and moves through a painful “freezing” stage (6 weeks to 9 months), a stiff “frozen” stage (4 to 6 months) and a “thawing” stage (6 months to 2 years)[31]. The NHS notes the pain can be worse at night and advises keeping the shoulder moving with your exercises[32]. In bed, rest the arm on pillows rather than forcing it into position.

Shoulder osteoarthritis

It usually affects people over 50, and night pain is common[12]. If it comes to surgery, sleep tends to recover: 2 to 5 years after shoulder replacement, 80% of 232 patients reported no sleep disturbance[15].

How to sleep after shoulder surgery

Follow your surgeon's instructions. Expect to sleep on your back, in a sling, for the first weeks, sometimes propped up. OrthoInfo notes that lying flat doesn't affect healing but may pull on the shoulder, so some patients sleep in a reclining chair or propped up in bed at first. Your surgeon decides how long the sling stays on[19]. A 2026 review adds sleeping on the uninjured side and positional pillows, though the evidence behind that advice is limited and indirect[20].

In a 2026 study of 50 patients after rotator cuff repair, back sleeping rose from 39% to 85% at two weeks, and side sleeping fell from 65% to 28%. Sleep was worse at two weeks, better than before surgery by six weeks, and most people were back in their usual position by six months. People who side-slept before surgery reported more pain-related waking[33], so practicing a supported back position beforehand may be worth a try, though nobody has tested it.

  • Elbow supported: if your surgeon allows it, a pillow or folded blanket under the arm keeps the elbow from dropping, as Cleveland Clinic suggests for back sleepers with rotator cuff pain[13].
  • Back to side sleeping: ask your surgeon when, then start on the uninjured side with a hug pillow.

Sleep keeps improving for months. Across 18 studies of shoulder replacement (2,512 patients), it improved little in the first six weeks and markedly by six months[34].

Is it your pillow or your shoulder?

If the ache shows up mainly when you wake, start with your setup. If it also hurts during the day, get the shoulder checked. Waking pain is common: 27% of 812 adults in one survey reported waking pain between the shoulder blades or in the arm[35], and in the trial above its frequency varied by pillow[21]. Signs that point beyond the pillow:

  • Pain or stiffness when reaching overhead[5].
  • Weakness lifting or rotating the arm[11].
  • Stiffness that makes the shoulder hard to move[32].
  • Pain that keeps waking you after you've changed side and pillow.

Test one change at a time for about a week. Shoulder self-care usually takes about two weeks to start working, and recovery can take six months or longer[29].

When shoulder pain needs medical attention

See a doctor if shoulder pain is getting worse or hasn't improved after two weeks, or if it's very hard to move your arm. Get urgent help for any of the signs below[29].

Frequently asked questions

What pillow is best for side sleepers with shoulder pain?

A pillow with a shoulder cutout and adjustable loft, like the Coop Original Crescent or Eli & Elm, plus a second pillow to hug for the top arm.

Should I sleep on my painful shoulder or the other side?

Generally the other side or your back, with the sore arm supported on a pillow. Side lying increases rotator-cuff pressure[4].

Why does my shoulder hurt more at night?

Night pain is typical of rotator cuff problems and shoulder arthritis[5,12], and lying on the shoulder adds pressure. If it keeps waking you, see a clinician.

Does hugging a pillow help shoulder pain?

Yes, for the top shoulder. It stops the top arm falling forward and rolling the shoulder in[6].

Can a pillow that is too low cause shoulder pain?

It can add to it. A low pillow lets your head drop, shifts weight onto the down shoulder, and tempts you to slide an arm underneath.

How do I stop my arm going numb when side sleeping?

Keep the arm out from under your head with enough loft or a cutout pillow, keep the elbow and wrist fairly straight[24,26], and hug a pillow with your top arm.

How should I sleep after rotator cuff surgery?

Follow your surgeon's instructions. Expect a sling, and some people sleep propped up in a recliner or on pillows for the first few days[19]. In one study, 85% slept on their back at two weeks, and most were back in their usual position by about six months[33].
